Thigh Pain in Cementless Total Hip Arthroplasty

Abstract
Purpose: This study was undertaken in order to identify the chronological changes in the prevalence of thigh pain, and its contributing factors, in cementless total hip arthroplasty.

Materials and Methods: Seventy five total hip arthroplasties were performed using an Anatomic stem and a harris-Galante cup. The mean age of the patients and follow-up period were 48.5 and 7.2 years, respectively. Both the clinical and radiographical parameters were evaluated.

Results: At the final follow-up, thigh pain had occurred in seventeen hips(22.7%). The prevalence of thigh pain was constant for four years postoperatively. However, the thigh pain rate increased abruptly at five years postoperatively. The thigh pain rate was positively correlated with the radiolucency at the porous pad of the stem and with the absence of a hydroxyapatite coating at the short term follow-up, and proximal osteolysis, due to excessive wear of the polyethylene, at the intermediate follow up.

Conclusion: Thigh pain can be reduced by choosing a properly designed stem for the stable mechanical and biological fixation, and a precise implantation technique, at the short follow-up. However, the adoption of a good surface bearing system for the reduction of wear is mandatory for the long term success.
